Evaluating indwelling devices and other risk factors for mortality in invasive Carbapenem-resistant Enterobacterales infections in Georgia, 2012–2019

Objective:
Carbapenem-resistant Enterobacterales (CRE) infections are a public health threat due to the risk of transmission between patients and high associated mortality. We sought to identify risk factors for mortality in patients with invasive CRE infections and to specifically evaluate whether there was an association between indwelling medical devices and 90-day mortality.
Design:Retrospective observational cohort study of patients infected with CRE in the eight-county metropolitan Atlanta area between 2012 and 2019.
Methods:Patients with invasive CRE infections were identified via the Georgia Emerging Infections Program’s active, population- and laboratory-based surveillance system and linked with the Georgia Vital Statistics database. We used bivariate analysis to identify risk factors for mortality and completed log binomial multivariable regression to estimate risk ratios (RR) for the association between indwelling devices and mortality.
Results:In total, 154 invasive CRE infections were identified, with indwelling devices present in most patients (87.7%) around the time of infection. Admission to an intensive care unit was found to be associated with 90-day mortality (adjusted RR [aRR] 1.55, 95% CI 1.07, 2.24); however, the presence of any indwelling device was not associated with increased risk of 90-day mortality in multivariable analysis (aRR 1.22, 95% CI 0.55, 2.73). Having at least two indwelling devices was associated with increased mortality (aRR 1.79, 95% CI 1.05, 3.05).
Conclusions:Indwelling devices were prevalent in our cohort but were not consistently associated with an increased risk of mortality. Further studies are needed to examine this relationship and the role of device removal.

Objective:
To determine whether residing in a hospital bed that previously held an occupant with Clostridioides difficile increases the risk of hospital-onset C. difficile infection (HO-CDI).
Methods:In this retrospective cohort study, we used a real-time location system to track the movement of hospital beds in 2 academic hospitals from April 2018 to August 2019. We abstracted patient demographics, clinical characteristics, and C. difficile polymerase chain reaction (PCR) results from the medical record. We defined patients as being exposed to a potentially “contaminated” bed or room if, within the preceding 7 days from their HO-CDI diagnosis, they resided in a bed or room respectively, that held an occupant with C. difficile in the previous 90 days. We used multivariable logistic regression to determine whether residing in a contaminated bed was associated with HO-CDI after controlling for time at risk and requiring intensive care. We assessed mediation and interaction from a contaminated hospital room.
Results:Of 25,032 hospital encounters with 18,860 unique patients, we identified 237 cases of HO-CDI. Exposure to a contaminated bed was associated with HO-CDI in unadjusted analyses (odds ratio [OR], 1.8; 95% confidence interval [CI], 1.4–2.31) and adjusted analyses (OR, 1.5; 95% CI, 1.2–2.0). Most of this effect was due to both mediation from and interaction with a contaminated hospital room.
Conclusions:Residing in a hospital bed or room that previously had a patient with C. difficile increases the risk of HO-CDI. Increased attention to cleaning and disinfecting the healthcare environment may reduce hospital transmission of C. difficile.

Background: Carbapenemase genes in carbapenem-resistant Enterobacterales (CP-CRE) may be transmitted between patients and bacteria. Reported rates of carbapenemase genes vary widely, and it is unclear whether having a carbapenemase gene portends worse outcomes given that all patients with CRE infections have limited treatment options. Methods: Using active population- and laboratory-based active surveillance data collected by the US CDC-funded Georgia Emerging Infections Program from 2011 to 2020, we assessed the frequency of carbapenemase genes in a convenience sample of CRE isolates using whole-genome sequencing (WGS), and we investigated risk factors for carbapenemase positivity. Only the first isolate per patient in a 30-day period was included. We compared characteristics of patients with CP-CRE and non–CP-CRE. Using multivariable log binomial regression, we assessed the association of carbapenemase gene positivity and 90-day mortality. Results: Of 284 CRE isolates, 171 isolates (60.2%) possessed a carbapenemase gene (Table 1), and KPC-3 was the most common carbapenemase gene (80.7%), with only 7 isolates possessing NDM (Table 2). No isolates possessed >1 carbapenemase gene, and most isolates were from urine (82.4%) (Table 1). Carbapenemase gene positivity was associated with lower age, male sex, black race, infection with Klebsiella pneumoniae, polymicrobial infection, having an indwelling medical device, receiving chronic dialysis, and prior stay in a long-term acute-care hospital, long-term care facility, and/or prior hospitalization in the last year. The 90-day mortality rates were similar in patients with non–CP-CRE and CP-CRE: 24.8% versus 25.7% (P = .86). In multivariable analysis, carbapenemase gene presence was not associated with 90-day mortality (adjusted risk ratio, 0.82; 95% CI, 0.50–1.35) when adjusting for CCI, infection with Klebsiella pneumoniae, and chronic dialysis use. Conclusions: The frequency of CP-CRE among CRE was high in this study, but unlike prior studies, the 90-day mortality rates wer similar in patients with CP-CRE compared to non–CP-CRE. Our results provide novel associations (eg, lower age, male sex, infection with Klebsiella pneumoniae, and indwelling medical devices) that infection preventionists could use to target high-risk patients for screening or isolation prior to CP-CRE detection.

Impact of exposure to potentially contaminated hospital beds on risk of hospital-onset C. difficile infection

Background: Environmental contamination increases risk for Clostridioides difficile infection (CDI) given that spores can remain on a hospital bed, floor, sink, and light switch despite appropriate cleaning measures. Using real-time asset management software (AgileTrac, GE Healthcare) for beds we examined the risk of a patient developing hospital-onset CDI (HO-CDI) when staying in a hospital bed that had a previous occupant with CDI. Methods: We retrospectively identified all patients in tracked beds from April 2018 to August 2019 to identify hospital-onset CDI (HO-CDI), defined as a positive PCR test for C. difficile in a patient hospitalized for >3 days. A patient was defined as being exposed to a potentially “contaminated” bed if within the preceding 7 days from their HO-CDI diagnosis they resided in a hospital bed that, within the prior 90 days, had held an occupant with CDI (Fig. 1). We used multivariable logistic regression to evaluate the association between being exposed to a contaminated bed and HO-CDI. Model covariates were chosen a priori based on known risk factors for CDI. As a sensitivity analysis, we varied the length of time that a bed could stay contaminated from 90 to 60, 30, 14, and 7 days. Results: We analyzed 25,032 hospital encounters representing 18,860 unique patients; we identified 237 (0.9%) hospital encounters with HO-CDI (Table 1). The Elixhauser comorbidity score, being exposed to a contaminated bed, and receiving antibiotics or a proton pump inhibitor (PPI) during the hospital admission were all associated with HO-CDI in the univariable analysis (Table 2). In the adjusted multivariable model, being exposed to a contaminated bed remained a significant risk factor for HO-CDI (OR, 1.60; 95% CI, 1.22–2.08) even after controlling for known risk factors for CDI including age >65, elevated Elixhauser score, and recent antibiotic or PPI use (Table 2). In the sensitivity analysis in which we adjusted the time a bed was considered contaminated after CDI, being exposed to a contaminated bed remained a risk factor for HO-CDI, with a similar odds ratios as the original model (Table 2). Conclusions: Residing in a hospital bed that contained a previous occupant with CDI is a risk factor for developing HO-CDI. Hospital epidemiologists, infection control personnel, and environmental services staff should consider this association when developing CDI risk mitigation strategies.

High seroprevalence of COVID-19 infection in a large slum in South India; what does it tell us about managing a pandemic and beyond?

People living in urban slums or informal settlements are among the most vulnerable communities, highly susceptible to coronavirus disease 2019 (COVID-19) infection and vulnerable to the consequences of the measures taken to control the spread of the virus. Fear and stigma related to infection, mistrust between officials and the population, the often-asymptomatic nature of the disease is likely to lead to under-reporting. We conducted a cross-sectional study to determine the seroprevalence of COVID-19 infection in a large slum in South India 3 months after the index case and recruited 499 adults (age >18 years). The majority (74.3%) were females and about one-third of the population reported comorbidities. The overall seroprevalence of IgG antibody for COVID-19 was 57.9% (95% CI 53.4–62.3). Age, education, occupation and the presence of reported comorbidities were not associated with seroprevalence (P-value >0.05). Case-to-undetected-infections ratio was 1:195 and infection fatality rate was calculated as 2.94 per 10 000 infections. We estimated seroprevalence of COVID-19 was very high in our study population. The focus in this slum should shift from infection prevention to managing the indirect consequences of the pandemic. We recommend seroprevalence studies in such settings before vaccination to identify the vulnerability of COVID-19 infection to optimise the use of insufficient resources. It is a wake-up call to societies and nations, to dedicate paramount attention to slums into recovery and beyond – to build, restore and maintain health equity for the ‘Health and wellbeing of all’.
Examining unique and prospective relationships among self-injurious thoughts and behaviors and posttraumatic stress disorder: a network analysis in two trauma-exposed samples

Background
As self-injurious thoughts and behaviors (SITB) remain a pressing public health concern, research continues to focus on risk factors, such as posttraumatic stress disorder (PTSD). Network analysis provides a novel approach to examining the PTSD-SITB relationship. This study utilized the network approach to elucidate how individual PTSD symptoms may drive and maintain SITB.
MethodsWe estimated cross-sectional networks in two samples of trauma-exposed adults (Sample 1: N = 349 adults; Sample 2: N = 1307 Veterans) to identify PTSD symptoms that may act as bridges to SITB. Additionally, we conducted a cross-lagged panel network in Sample 2 to further clarify the temporal relationship between PTSD symptoms and SITB during a 2-year follow-up. Finally, in both samples, we conducted logistic regressions to examine the utility of PTSD symptoms in prospectively predicting SITB, over a 15-day period (Sample 1) and over a 2-year period (Sample 2), allowing us to examine both short- and long-term prediction.
ResultsTwo PTSD symptoms (i.e. negative beliefs and risky behaviors) emerged as highly influential on SITB in both cross-sectional networks. In the cross-lagged panel network, distorted blame emerged as highly influential on SITB over time. Finally, risky behaviors, unwanted memories, and psychological distress served as the strongest predictors of SITB across the two samples.
ConclusionsOverall, our results suggest that treatments targeting negative beliefs and risky behaviors may prevent SITB in community and Veteran populations, whereas treatments targeting distorted blame and unwanted memories may help reduce SITB for individuals with a history of combat trauma.
Facilitating collaborative animal research: The development and implementation of a Master Reciprocal Institutional Agreement for Animal Care and Use

Ensuring appropriate review, approval, and oversight of research involving animals becomes increasingly complex when researchers collaborate across multiple sites. In these situations, it is important that the division of responsibilities is clear and that all involved parties share a common understanding. The National Institutes of Health Office of Laboratory Animal Welfare and the United States Department of Agriculture Animal Plant Health Inspection Service require an Institutional Animal Care and Use Committee (IACUC) to review the care and use of animals in research, and both agree that it is acceptable for one IACUC to review the work taking place at multiple institutions. With this in mind, several Harvard-affiliated hospitals and academic centers developed the Master Reciprocal Institutional Agreement for Animal Care and Use (Master IACUC Agreement) to support collaboration, decrease administrative burden, increase efficiencies, reduce duplicative efforts, and ensure appropriate protections for animals used in research. Locally, the Master IACUC Agreement has fostered greater collaboration and exchange while ensuring appropriate review and oversight of research involving animals. As multisite animal protocols become more prevalent, this Agreement could provide a model for a distributed, national network of IACUC reliance.
The SMART IRB platform: A national resource for IRB review for multisite studies

Single institutional review board (IRB) review of multisite research increased in frequency over a decade ago with a proliferation of master IRB reliance agreements supporting statewide and regional consortia and disease- and population-specific networks. Although successful, the increasing number of agreements presented significant challenges and illuminated potential benefits of a single, nationwide agreement. Anticipated changes in federal regulations highlighted the need to systematize and simplify IRB reliance. To address these challenges, the NIH National Center for Advancing Translational Sciences funded a project to establish a national IRB reliance network that would support national adoption of single IRB (sIRB) review. The Streamlined, Multisite, Accelerated Resources for Trials (SMART) IRB Platform launched in July 2016 to facilitate dissemination, adoption, and implementation of a collaboratively developed master IRB reliance agreement and supportive tools and resources. More than 580 institutions have joined SMART IRB’s Master Common Reciprocal Institutional Review Board Authorization Agreement and begun using the SMART IRB platform to support sIRB arrangements. Here, we describe the tenets of the agreement and operational benefits and challenges of its use. SMART IRB’s early success affirms the utility of collaborative, flexible, and centralized approaches to supporting sIRB review while highlighting the need for further national harmonization.

There is a paucity of educational resources for potential clinical trial participants, particularly resources in plain language, attentive to health literacy principles and translated into native languages. The New England Research Subject Advocacy Group was formed to explore common issues, interests, and concerns related to the experience of participation in clinical research and research participant safety. Specifically, the group sought to increase community awareness and trust through the development and distribution of publicly accessible informational resources. In support of these aims, the group developed a robust library of high-quality, plain-language educational materials covering topics in health research, research participation, and common research procedures, and translated the majority of the materials into an additional 15 languages. These resources have been downloaded over 130,000 times. After English, the most common languages downloaded are Vietnamese, Spanish, and Korean.

Summary
Introduction
Since ancient times, the evergreen carob tree Ceratonia siliqua L. (Fabaceae: Caesalpinoideae) has been grown in most countries of the Mediterranean basin for its edible seed pots, which are an important crop (von Hasselberg 2000). It has been used historically as feed for domesticated animals (sometimes referred to as “locust beans”), and the current cultivars of the carob tree were probably selected by the Arabs (Ramón-Laca and Mabberley 2004). Carob seed pots were also used to supplement the human diet (e.g. known as “St. John’s bread”) and its products are used even nowadays in many ways (e.g. as thickening agents). Carob trees were traditionally interplanted with olives, grapes, almonds, and barley in low-intensity farming systems in most carob-producing countries (Battle and Tous 1997). The carob tree is a large, sclerophyllous tree of the Mediterranean evergreen maquis (von Hasselberg 2000; Zohary and Orshan 1959). The tree is usually dioecious (hermaphrodites occur rarely: Zohary 1972: 32; Tucker 1992) and produces many-flowered catkin-like inflorescences (von Hasselberg 2000; Battle and Tous 1997; Feinbrun-Dothan and Danin 1998: 294) with strongly reduced flowers. The pentamerous flowers of both sexes are 6–12 mm long, yellowish–green, apetalous and consist merely of sexual organs (von Haselberg et al. 2004). Male flowers have five stamens and an abortive pistil, whereas female flowers have abortive staminodia and a fully developed pistil formed of a single carpel (Tucker 1992). The oval, two-lobed stigma is about 2.5 × 2.3 mm in size (von Hasselberg 2000), peltate, wet, and covered by verrucate papillae (Tucker 1992). The floral nectar produced is exposed (Battle and Tous 1997) on the broad hypogynous disk (Polhill et al. 1981) and therefore easily accessible for flower-visiting insects. The strongly scented inflorescences (Custodio et al. 2004, 2006), usually bearing 20–50 single flowers, arise as short lateral racemes mainly on branches (cauliflorous flowering) and on the trunk (ramiflorous flowering) (von Haselberg et al. 2004). The prolonged flowering season is mainly from September to December, which is regarded as a harsh pollination environment due to climatic conditions, a low diversity of potential pollinators, and a low number of individuals (Dafni 1986).
